For the purpose of providing services and/or merchandise to Gearvana LLC or any of the U.S. affiliates (“Gearvana”), APPLICANT agrees to authorize the bank and trade references to release credit and financial information. The following statements in writing are made by the APPLICANT, and Gearvana may rely on all such statements as correct.

APPLICANT acknowledges that it has read, understands and has agreed to the terms and conditions hereof and agrees to be bound by them, that this document with attachments is the complete and exclusive statement of the agreement between APPLICANT and Gearvana relating to the subject matter hereof and that this document supersedes all proposals, oral or written. APPLICANT further agrees to notify Gearvana in writing within five (5) days of any change of ownership, address, telephone, authorized purchasing agents, banks, transfer or listed assets, or other facts set forth in this application.

This document has to be signed by a person who is legally authorized to sign on behalf of the company.
